Make a photocopy of your state-issued business license. Online printouts and hand-filled forms will not be accepted. (If you are in the state of Texas, we need a copy of your Sales and use Tax Permit and a completed Resale Certificate https://s3.amazonaws.com/apmc/images/dlicon.png

Send in your application and business license to the fax or e-mail address listed at the bottom of the application. Please send in everything at once. Partial / incomplete submissions will delay processing application.

After submitting your documents, please be patient while we review your information and create your account. Please be sure you have already created an alexorthopedic.com account with the "Dealer / Re-seller" account type so that we can assign your dealer discounts.
